CANCELLATION POLICY
Our office has a 24-hour cancellation policy. If you need to cancel or reschedule, please contact us at least 24 hours prior to your appointment. If you are unable to cancel within 24 hours, you may be held responsible for your visit at a minimum fee of $75.
PUNCTUALITY POLICY
It is our priority to spend quality time with each patient, therefore we are unable to accommodate patients who are more than 20 minutes late. If you are more than 20 minutes late, we will not be able to treat you, and you may be held responsible for your visit at a minimum fee of $75. If you are less than 20 minutes late, we can proceed with treatment, but your session will be shortened by an equal duration.
COMMUNICATIONS POLICY
We want you to feel comfortable discussing your health with your practitioner. If you require more than the 10-minute intake allotted at follow-up visits, you may schedule an additional 15 minutes for a fee of $25. Alternatively, you can email a written inquiry to your practitioner. If your particular concern requires the practitioner to perform extensive evaluation, research or writing, you will be billed at a rate of $50 per half hour. Examples include, but are not limited to, reviewing lab results, evaluating herb and supplement protocols, making diet, nutrition or lifestyle recommendations, and writing medical letters.
NON-BUSINESS HOUR POLICY
Some patients, such as those undergoing IVF procedures, may require very specific timing of their acupuncture treatments. If there is a need to schedule your appointments outside our normal business hours, an additional $50 fee will apply.
PRIVACY POLICY
I have read and understand the Notice of Privacy Practices as required by the privacy regulations created as a result of the Health Insurance Portability and Accountability Act of 1996 (HIPAA). The complete privacy notice will be provided to me upon my request.
